Qualifing Statements

• I will collect and retain information on honorees as needed to certify their personal eligibility for the award as well as the completion of eligible service.
• I will only provide true and accurate information on this application and will maintain accuracy of information on my organization’s profile.
• My organization will cover the costs of the honorees’ awards and will not pass costs onto volunteers or request donations, other than general member dues, as a condition of recognition.
• My volunteers’ participation with our program does not encourage or provide instructions for a criminal offense, violate the human rights of any individual, or violate any local, state, or international law.
