Is the organization based in the U.S., its territories, or an overseas U.S. military installation?

Certifying Organizations must receive or facilitate volunteer service. This is defined as:

Receive:
The organization engages volunteers to support their own programs, goals, or events. For example, a community association uses volunteers to maintain the community garden.
Facilitate:
The organization encourages members to volunteer at other organizations and keeps a record of the volunteer service including hours completed. For example, a service fraternity whose members complete volunteer hours each year or a business’ employees who volunteer in their local community. The organization matches individuals to volunteer opportunities. For example, an organization manages a volunteer matching program or platform.

Do you receive or facilitate volunteer service?

Please read and attest to your understanding and agreement with the following statements:

I will collect and retain information on honorees as needed to certify their personal eligibility for the award as well as the completion of eligible service.
I will only provide true and accurate information on this application and will maintain accuracy of information on my organization’s profile.
My organization will cover the costs of the honorees’ awards and will not pass costs onto volunteers or request donations, other than general member dues, as a condition of recognition.
My volunteers’ participation with our program does not encourage or provide instructions for a criminal offense, violate the human rights of any individual, or violate any local, state, or international law.
